Office Policies

Identification/Insurance Cards

All patients are asked to present a valid form of identification such as drivers license, picture identification card, passport, or military identification card. Patients will also need to present their current insurance card. It is the patient's responsibility to bring current insurance information and necessary referrals in order to be seen.

Treating Minor Children

The first visit of any patients younger than 18 years old MUST be accompanied by their parent or legal guardian. After the first visit, the parent may provide written authorization allowing a designated individual to escort their child.

Payments

Co-pay and payment for cosmetic procedure are required at the time of service. If your insurance plan has coinsurance or unmet deductible, please be ready to provide contingent credit card authorization for patient responsibilities or leave a deposit on your visit. Forms of payment accepted include cash, paypal, apple pay, google pay and all major credit/debit cards.

General Cancellation Policy

If you need to cancel or change an appointment, please kindly contact our office to do so at least 24 hours before your scheduled appointment. This courtesy will make it possible to give your appointment time to another patient.

Cosmetic and Surgical Cancellation Policy

It is necessary for us to reserve longer appointment times for surgical and cosmetic procedures. If you need to cancel or change an appointment, please kindly contact our office to do so at least 24 hours before your scheduled appointment.

In Office Biopsy

Whenever a lesion is removed from the skin, the tissue will be automatically sent for pathology. This will result in two separate charges. One will be from your dermatologist for removing the sample. The other will be from the pathologist for examining the sample under the microscope.
